Description

This unit is designed to assist students to understand the role of the nurse in long term health condition management, primarily within a primary health care setting. This includes patient self-management, early detection of health conditions, and health promoting strategies which may improve the sequelae of long-term health conditions and improve patient outcomes.

Learning Outcomes

On completion of this unit students should be able to:

  1. Critically define strategies which promote population health.
  2. Analyse a range of strategies to support individuals with long term health conditions.
  3. Articulate the role of the nurse in long term management of individuals with chronic health conditions.
  4. Appraise the role of the multi-disciplinary team in the management of individuals with chronic health conditions.
  5. Develop strategies to reduce the impact of chronic health conditions upon individuals and their families.

Unit Content

  1. Chronic health condition self-management including goal setting.
  2. Impact of chronic health conditions on physical functioning, interpersonal relationships and emotional well-being.
  3. Identifying health behaviours and barriers to change.
  4. Reducing the impact of chronic health conditions.
  5. Professional, legal and ethical issues including end of life care.
  6. The health care burden of chronic health conditions.
  7. High risk populations.

Learning Experience

Students will attend on campus classes as well as engage in learning activities through ECU Blackboard.
