* These are mandatory requirements for supervised placements including but not limited to HST3502 (Health Sciences Professional Practices).

** Supervised placements occur from Year 2.

The School of Medical and Health Sciences requires that your pre-placement preparation documentation is up-to-date at the start of each semester and remains valid for the duration of your placement(s).

  • Hand Hygiene Certificate
  • Mental Health First Aid
  • Risk Management Checklist
  • Student Declaration

Recommended

  • Applied First Aid (HLTAID011)
  • COVID-19 Vaccination

Placement Site Specific Requirements

  • Hepatitis A Immunisation
  • Working with Children Check
  • National Police Clearance
  • WA Department of Health National Criminal History Record Check
  • Some placement sites will require completion of their own specific pre-placement preparation documentation.  You will be notified of requirements in SONIA (SMHS Placement Management System) or by your Placement/Unit Coordinator.
  • Student Travel Approval Form (rural/remote placements).
  • Mask Fit Testing Certificate.
